If you mean fail to defend as in drop out of graduate school at some point after you've passed all the exams and before you get to defend your dissertation, then also yes. This scenario is way more common than it should be. I know of at least 3 people that got to year 3.5-4.5 and decided to leave with a masters. I know of one guy at my school who got to the end of year 6, was told to write and just never did. I know of one guy from my group who got to year 10.5, was told to write, passed his defense by some miracle and then STILL failed to get his PhD. He never turned in his corrections.
